• Assist with preadmission services and discharge planning, linking to community resources.
• Intervene in Advance Care Planning and ensure documentation in medical records.
• Attend Care Plan meetings and participate in quality committees if requested.
• Maintain confidentiality and comply with policies, regulations, and training.
