Job summary

Preferred Care Health Centers in Potomac, MD seeks a qualified Social Worker to guide the Social Services Department in delivering person-centered care. You will assess needs, coordinate services, and ensure compliance with state and federal regulations.

Reporting to the Social Services Director, you will advocate for residents, participate in care planning, and support families while safeguarding residents’ rights and dignity.

Qualifications

• Bachelor’s degree in social work required.
• Licensed in Maryland required.
• Minimum of 1 year of supervised social work experience in a health care setting.

Responsibilities

• Oversee development, implementation, supervision and evaluation of the Social Services Department to meet residents' needs and comply with state/federal regulations.
• Complete social services assessments and contribute to care plans
• Advocate for residents and assist with discharge planning and accessing community resources
• Document social work actions in medical records and uphold professional standards
• Engage in Advance Care Planning and ensure residents’ rights and preferences are respected
• Coordinate with other departments and participate in care plan meetings and quality committees
