Aventura Healthcare Group is seeking a Licensed Social Worker (LSW) to provide professional social work services to residents in a long-term care facility. The LSW will support emotional, psychosocial, and environmental needs while ensuring compliance with federal and state regulations and promoting residents’ rights and quality of life.

Responsibilities include psychosocial assessments, care planning, family counseling, discharge planning, advocacy, and compliance with CMS and facility

Qualifikationen

• Bachelor’s degree in Social Work (BSW) is required.
• Current state licensure as a Licensed Social Worker (LSW) in good standing.
• 1–2 years of healthcare or long-term care experience is preferred.
• Knowledge of long-term care regulations and resident rights; strong documentation and assessment skills.

Aufgaben

• Conduct comprehensive psychosocial assessments upon admission and periodically.
• Develop, implement, and update individualized care plans with the interdisciplinary team.
• Provide counseling and emotional support to residents and families; manage crisis situations.
• Assist with discharge planning and transitions to appropriate care settings; coordinate referrals.
• Advocate for residents’ rights and ensure CMS/state compliance; participate in quality-improvement initiatives.
