Job summary

Fundamental is seeking a Licensed Social Worker (LSW) to oversee the Social Services Department in a facility in Denison, Texas. You will coordinate with discharge planners and families to arrange tours and follow-up, while ensuring compliance with federal, state, and local standards.

The role requires a current Social Work license, a Bachelor’s in Social Work is preferred, and at least 3+ years in a long-term care setting.

Qualifications

• Must hold current Social Work license in good standing.
• Bachelor’s Degree in Social Work preferred.
• Minimum 3+ years of experience in a long term care facility.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct Social Services within the facility.
• Lead initial contact with discharge planners and families to arrange tours and follow-up.
• Develop and implement policies for social and emotional patient needs.
• Build strong working relationships with internal departments and external agencies.
• Stay current on federal and state regulations and professional standards.
