Job summary

Spring Grove Rehabilitation and Health Care Center is seeking a Licensed Social Worker with SNF/LTC experience to support residents and families through psychosocial assessment, care planning, and coordination of services. You will collaborate with interdisciplinary teams to promote well-being and quality of life in a 106-bed Sub-Acute and Long-Term Care setting.

Qualifications

• Bachelor’s degree required; Master’s degree strongly preferred.
• 1+ years of experience as a Social Worker in a healthcare-related setting required; nursing home or rehabilitation center strongly preferred.
• Must have CSW/LCSW/LSW certification.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff, and external agencies.
• Ability to work effectively in a fast-paced, team-oriented healthcare environment.

Responsibilities

• Evaluate residents to assess psychosocial needs, strengths, and limitations.
• Address resident concerns, goals, and emotional needs while communicating with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.
