Job summary

RegalCare at Wakefield is seeking a Social Service Director to lead the social services team, overseeing resident assessments, care planning, counseling, and rights education. The role collaborates with interdisciplinary staff to support residents and families.

The position coordinates discharge planning, hospital liaisons, and admissions processes, while advocating for residents and ensuring CMS and regulatory compliance.

Responsibilities

• Complete initial psychosocial assessments, Minimum Data Set (MDS) portions, and ongoing progress notes.
• Develop and update individualized resident care plans in collaboration with the interdisciplinary team.
• Provide emotional support and counseling to residents and families dealing with illness, life changes, grief, loss, or depression.
• Assist residents and families with advanced directives, living wills, and understanding Residents' Rights.
• Coordinate discharge planning, home-health referrals, and community resources to transition residents safely.
• Collaborate with hospital liaisons and the business office regarding admissions, insurance verification, and room changes.
• Act as an advocate for resident wishes and coordinate investigations into resident grievances.
• Ensure the department complies with state, local, and federal regulations (such as CMS guidelines).
• Coordinate logistics for family council meetings and staff education on resident rights.
