Job summary

Milestone Senior Services is seeking a Licensed Social Worker to manage Medicaid MI Choice Waiver clients across Kalamazoo and nearby counties. You will assess, plan, link, monitor, and advocate for services while collaborating with internal staff and external providers.

The role requires an MSW or BSW with MI licensure and at least one year of relevant experience, with a hybrid 50/50 in-person/remote schedule. Benefits include PTO and holidays.

Qualifications

• BSW or MSW with State of Michigan licensure.
• MSW preferred.
• At least one year of relevant work with elderly or in-home services.

Responsibilities

• Conduct comprehensive assessments to determine eligibility and care needs.
• Develop plans of care with frequency and duration of services.
• Link, coordinate, and monitor services through providers.
• Document assessments and progress per policy and procedures.
• Collaborate with nurses and the care management team.
• Prepare reports for service delivery, caseload tracking, and feedback.
• Participate in team meetings and coordinate with agency/community resources.
• Provide staff education and support to service providers.
