Job summary

Area Agency on Aging 1-B in Michigan is seeking a SW Supports Coordinator to perform comprehensive assessments for MI Choice Waiver eligibility and coordinate long-term care plans. The role includes linking participants to services and monitoring progress.

The position requires a MI social work license, a BSW or MSW, and experience with psycho-social assessments. Up to 50% travel within the MI Choice area is expected.

Qualifications

• Bachelor's or Master’s degree in social work and MI licensure.
• Experience in psycho-social assessment or home care is preferred.
• Strong time management and ability to work with diverse populations.

Responsibilities

• Perform comprehensive SW assessments for MI Choice eligibility.
• Coordinate LTC plans and link participants to services.
• Conduct case transfers and maintain participant records.
• Participate in meetings, trainings, and QA activities.
• Travel within a multi-county MI Choice program area as needed.
