Job summary

AgeWays is seeking a licensed social worker to perform comprehensive MI Choice assessments and coordinate person-centered care plans. The role requires travel across counties within the MI Choice program area and maintaining accurate records in the electronic medical record system.

The candidate will assess medical, functional, psychological, financial, and environmental needs and facilitate service coordination to support long-term care planning for participants.

Qualifications

• Must have a MSW or BSW with Michigan licensure (LLMSW/LLBSW).
• Experience in psycho-social assessment, home care, or similar populations preferred.
• Strong time management and ability to work independently in a community-based setting.

Responsibilities

• Conduct dual-discipline in-person SW assessments for MI Choice eligibility.
• Coordinate support services and develop a Person-Centered Service Plan (PCSP).
• Enter data into electronic medical records and maintain participant files.
• Monitor participants through calls and meetings; participate in QA activities.
• Travel within the MI Choice region to cover multiple counties as needed.
