Job summary

Brentwood Place Three is seeking a Licensed Social Worker to assess and address psychosocial needs of residents in a geriatric facility. The role follows NASW codes of ethics and regulatory guidelines while supporting residents’ rights and family education.

You will collaborate with an interdisciplinary team to develop and implement care plans, coordinate discharges, and assist in transitions with appropriate referrals. This is a full-time, on-site position with competitive wages and benefits.

Qualifications

• Certified, licensed, or registered in the state of practice.
• Bachelor’s Degree in Social Work or Human Services field from an accredited school.
• One year of supervised social work experience in a healthcare setting with geriatric individuals.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment identifying social, emotional, and psychological needs.
• Maintains documentation compliant with federal/state regulations, policies, and care plans.
• Supports resident rights and informs about therapeutic alternatives to refused services.
• Participates in developing an interdisciplinary care plan addressing psychosocial needs.
• Provides therapeutic interventions to aid residents' adjustment to long-term care.
