Job summary

Brentwood Place Three is seeking a Licensed Social Worker to identify psychosocial needs and assist in accessing services for residents in our Dallas area facility. The role requires NASW-aligned practice, compliance with federal/state regulations, and compassionate, resident-centered care.

You will conduct social histories, contribute to care plans, document thoroughly, support families, coordinate discharge planning, and collaborate with the nursing and administrative teams.

Qualifications

• Certified, licensed, or registered in state of practice.
• Bachelor’s Degree in Social Work or Human Services field from an accredited school.
• One year of supervised social work experience in a healthcare setting with geriatric clients.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident identifying social, emotional, and psychological needs.
• Maintains accurate and timely documentation per regulations and policy.
• Supports resident rights and educates families; documents refusals and alternatives.
• Participates in development of an interdisciplinary care plan.
• Ensures therapeutic interventions for adjustment to long-term care.
• Provides support and education to residents/families about placement and services.
• Coordinates discharge planning and referrals for home care services.
• Collaborates with the team to protect resident well-being and rights.
• Assists in inspection preparations and responds to survey findings.
• Other duties as assigned.
