Outfield-Healthcare-Partners is seeking a licensed social worker to assess and address residents' psychosocial, emotional, and cultural needs in a skilled nursing setting. You will complete social histories, contribute to care plans, and coordinate with an interdisciplinary team to support resident well-being.

The role requires a state license, a BA in Social Work or related field, and at least one year of geriatric social work in healthcare; nursing experience is a plus.

Competenze

• Must be licensed/registered in the state of practice.
• Bachelor's degree in Social Work or Human Services field required.
• One year of supervised social work experience with geriatric population.
• Two years nursing experience in a Skilled Nursing Facility preferred.

Mansioni

• Completes social history and psychosocial assessment identifying social, emotional, and psychological needs; contributes to the care plan.
• Maintains compliant documentation per federal/state regulations and center policies (MDS, care plans, progress notes).
• Supports resident self-determination; documents refusals and educates on alternatives.
• Participates in development of an interdisciplinary care plan addressing psychosocial needs.
• Provides therapeutic interventions to aid adjustment to long-term care facility.
• Offers support/education to residents and families about placement and services; refers to social service agencies when needed.
• Coordinates discharge planning and referrals for home care services prior to community return.
• Collaborates with the interdisciplinary team to promote resident rights and psychosocial well-being.
