Job summary

St. Elizabeth Physicians seeks an LSW to join the Care Management – Integrative Care Team. The social worker will access community resources, navigate patients through complex systems, and perform face-to-face visits in offices, Medicaid venues, libraries, or homes.

The role emphasizes collaboration with physicians, nurses, and care coordinators to ensure culturally sensitive, patient-centered care and timely communication.

Qualifications

• Licensed Social Worker in Kentucky (LSW) or eligible (KY/IN).
• BSW degree from an accredited program.
• 0–3 years of relevant social work experience.
• Ability to coordinate resources and work with a care team.

Responsibilities

• Manage a caseload of high-risk patients and coordinate social needs.
• Conduct face-to-face visits in PCP offices, community settings, or home.
• Document visits, plans, and interventions in the medical record.
• Educate patients on community resources and supports.
• Coordinate with physicians and care teams to improve outcomes.
