Job summary

The LSW provides population health support by connecting patients to community resources and conducting face-to-face encounters in primary care offices, Medicaid offices, libraries, and patients' homes. Qualifications

• BSW degree accredited program.
• Licensed Social Worker (LSW) in KY and IN per state requirements.
• Experience in social work within healthcare or community settings is preferred.

Responsibilities

• Comply with all applicable laws and regulations.
• Provide patient-centered evaluation, care planning, and resource coordination during home visits.
• Coordinate with physicians, nurses, and community partners to meet social needs.
