Job Requirements

• Education
  • Bachelor’s degree in Social Work (BSW) from an accredited program required. Master’s degree in Social Work (MSW) preferred. LMSW or appropriate social work licensure/certification required within 18 months of employment, consistent with applicable state requirements.
• Experience
  • Knowledge of social determinants of health, care coordination, community resources, and behavioral health concepts.
• Physical

  • Normal clinic environment. Hearing of normal and soft tones. Frequent sitting, standing, walking, climbing and reaching. Lifting and carrying up to 15 lbs. Uses computer, typewriter, fax machine and telephone.

Ability to manage multiple priorities and work with individuals experiencing complex social, medical, and behavioral health needs.

Proficiency with electronic health records and standard computer applications preferred.

Experience in healthcare, population health, care management, behavioral health, community health, or social services preferred.

Experience working with value-based care models, accountable care organizations, managed care, or other population health programs preferred.

The LMSW will work closely with patients, families, providers, care management teams, community organizations, and other healthcare professionals to develop individualized interventions and connect patients with appropriate medical, behavioral health, and community resources.
