Qualifications

• BSW degree from an accredited program.
• 0–3 years of experience.
• Licensed Social Worker in Kentucky and Indiana.

Responsibilities

• Comply with all applicable laws and regulations.
• Provide patient-centered evaluation, care planning, and resource coordination during home visits.
• Document visits and communications in EPIC in a timely manner.
• Coordinate with healthcare teams and community resources to meet patient needs.
