Job summary

Ohio Department of Behavioral Health in Cincinnati, OH is seeking a permanent, full-time social work professional for Summit Behavioral Healthcare. You will develop individualized treatment plans with patients and the interdisciplinary team, monitor progress, document outcomes, and coordinate with courts and external agencies.

The role requires an LISW license, 40-hour work week with weekend rotation, and is located at Summit Behavioral Healthcare Campus, 1101 Summit Road, Cincinnati, OH 45237.

Qualifications

• Requires current license as independent social worker (i.e., LISW) as issued by the State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board.
• Pursuant to ORC 4757.41(A)(5) any person with two years of service as of July 10, 2014 is exempt from current licensure requirements.

Responsibilities

• Develop, implement, and regularly review individualized treatment plans with patients and the interdisciplinary team.
• Monitor patient progress and modify interventions based on clinical needs and response.
• Document clinical services assessments, interventions, and patient progress accurately and timely.
• Identify patient strengths, treatment needs, risk factors, barriers to discharge and available support.
• Collaborate with legal representatives, courts, attorneys, probation/parole personnel and other authorized agencies as appropriate.
• Assist with evaluations and treatment activities related to competency restoration or other forensic requirements.
• Maintain awareness of court orders, legal requirements and patient rights affecting treatment and discharge planning.
• Develop and sustain collaborative relationships with external facilities to facilitate referrals and follow-up.
