Legacy Hospice, Inc. in Jackson, MS is seeking a compassionate Social Worker to join our interdisciplinary team. You will assess patients and families, provide counseling on coping with terminal illness, and help with advance care planning and practical arrangements.

The role emphasizes cultural sensitivity, crisis intervention, collaboration with care teams, and education of staff and families. A BSW is required, MSW/LCSW preferred, with CPR and a valid driver's license.

Qualifications

  • BSW required; MSW or LCSW strongly preferred.
  • At least 1 year of social work experience, preferably in hospice, home health, or a community setting.
  • Knowledge of end-of-life care, grief support, and cultural sensitivity.
  • Strong written and verbal communication skills.
  • Current CPR certification; valid driver's license and reliable vehicle.

Responsibilities

  • Conduct psychosocial assessments of patients and caregivers.
  • Provide counseling to patients and families regarding emotional stress, coping, grief, and advance care planning.
  • Assist with advance directives, funeral arrangements, and navigating legal/financial decisions.
  • Identify and coordinate with community resources, agencies, and support organizations.
  • Educate staff and families about cultural and psychosocial needs.
  • Document patient conditions, interventions, and psychosocial support provided.
  • Serve as a liaison between families and care teams to ensure coordinated, holistic care.
  • Participate in interdisciplinary team meetings to develop individualized care plans.
  • Provide crisis intervention and support for safety concerns (neglect, abuse, self-harm).
  • Participate in quality improvement initiatives and continuing education.
  • Participate in on-call support as needed.

Qualifications
  • Bachelor's degree in Social Work (BSW) required
  • Master's degree (MSW) or LCSW strongly preferred
  • At least 1 year of experience in social work, preferably in hospice, home health, or a community setting
  • Knowledge of end-of-life care, grief support, and cultural sensitivity
  • Strong written and verbal communication skills
  • Self-motivated, organized, and able to work independently
  • Current Basic Life Support (CPR) certification
  • Valid driver's license, auto insurance, and reliable vehicle required
