Join our Team

We are currently expanding our team and are looking for a Licensed Social Worker. As a member of our team, your compassionate care, communication skills, and understanding of the psychosocial dynamics of illness, loss, and death will be instrumental in providing direct patient care during the final stages of life.

Key Responsibilities
  • Assess the psychosocial status of patients and their families/caregivers related to the patient’s terminal illness and environment.
  • Communicate any special needs to the interdisciplinary team.
  • Evaluate family dynamics, caregiver abilities, volunteer and bereavement needs, etc., and provide interventions as necessary.
  • Provide information to patients, families, caregivers, and community agencies.
Key Qualifications
  • Meet state-specific guidelines, education, and licensure requirements. Meets personnel qualifications stated in 418.114(b)(3).
  • Minimum of one (1) year of experience in healthcare; hospice experience preferred.
  • Understand hospice philosophy and issues of death/dying.
  • Demonstrate good verbal and written communication and organization skills.
  • Possess and maintain current CPR Certification.
  • Must be licensed, have insured transportation, and maintain a vehicle in good working order.
