Job summary

Legacy Hospice, Inc. in Batesville, AR is seeking a compassionate Hospice Social Worker to join our care team.

You will conduct psychosocial assessments, provide counseling, assist with advance directives, and coordinate with families and community resources to support patients with a terminal diagnosis. At Legacy Hospice were committed to caring for patients and families with compassion and dignity We are seeking a skilled and empathetic Social Worker SW to join our team In this vital role youll help patients and families cope with the emotional social and practical challenges that come with a terminal diagnosis bringing comfort and clarity during their most vulnerable moments

What You’ll Do

• Conduct psychosocial assessments of patients and caregivers
• Provide counseling to patients and families regarding emotional stress coping grief and advance care planning
• Assist with advance directives funeral arrangements and navigating legalfinancial decisions
• Identify and coordinate with community resources agencies and support organizations
• Educate staff and families about cultural and psychosocial needs
• Document patient conditions interventions and psychosocial support provided
• Serve as a liaison between families and care teams to ensure coordinated holistic care
• Participate in interdisciplinary team meetings to develop individualized care plans
• Provide crisis intervention and support for safety concerns neglect abuse self harm
• Participate in quality improvement initiatives and continuing education
• Participate in on call support as needed

Qualifications

• Bachelors degree in Social Work BSW required
• Masters degree MSW or LCSW strongly preferred
• At least 1 year of experience in social work preferably in hospice home health or a community setting
• Knowledge of end of life care grief support and cultural sensitivity
• Strong written and verbal communication skills
• Self motivated organized and able to work independently
• Current Basic Life Support CPR certification
• Valid drivers license auto insurance and reliable vehicle required
