Job summary

Icare-Health-Networ is seeking a Social Work Director to coordinate and supervise social work services across the facility, ensuring care aligns with state and federal standards. The role requires directing staff, performing resident assessments, and overseeing psychosocial support for residents and families.

The position emphasizes collaboration with the treatment team, compliance with regulations, and development of new behavioral and psychosocial programs to enhance resident well-being.

Qualifications

• Master’s Degree in Social Work (MSW) preferred.
• Bachelor’s Degree in Social work or related human services field, required.
• A minimum of one years’ experience in long term care providing direct social services, required.
• Experience behavioral health preferred.

Responsibilities

• Supervise the facility's Social Work Services department and contracted staff.
• Conduct face-to-face resident interviews and assessments of resident cognitive, mood, behavioral status and other psychosocial factors.
• Plan for resident discharges.
• Respond to clinical emergencies, regulatory visits, surveys and issues outside of normal work hours as needed.
• Direct and supervise the delivery of medically related social services to all residents and their families.
• Collaborate with the Staff Development Coordinator and Administrator to provide staff training in Residents Rights and Behavioral Health issues.
• Participate in the clinical evaluation and screening of potential admissions to the facility.
• Collaborate with the interdisciplinary treatment team to access community-based services for residents.
• Serve as liaison with contracted mental health practitioners and consultants.
• Develop and implement new behavioral and psychosocial service programs.

Job description

Social Work Director

The primary role of the Social Work Director is to coordinate, supervise and facilitate the delivery of Social Work Services, as provided by facility or facility contracted staff, to all residents of the Facility, and in accordance with applicable state and federal regulations and professional standards. This position fulfills the responsibility of direction of the facility’s social work services as required by the CT Public Health Code.

Responsibilities

• Supervise the facilities Social Work Services department and facility contracted staff consistent with operational guidelines.
• Conduct face to face resident interviews and assessments of resident cognitive, mood, behavioral status in addition to other psychosocial factors.
• Participate in planning for resident discharges.
• Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed.
• Direct and supervise the delivery of medically related social services to all residents of the facility, as well as their family members and significant others as needed.
• Collaborate with the facility Staff Development Coordinator and Administrator to provide required and as necessary staff training in Residents Rights and Behavioral Health issues.
• As needed, participate in the clinical evaluation and screening of potential admissions to the facility.
• To work in collaboration with the facility interdisciplinary treatment team in accessing information regarding, and as appropriate making referral to, community based services for residents who wish to access such services.
• To serve as a facility treatment team liaison with facility contracted mental health practitioners and consultants.
• Actively participates in developing and implementing new behavioral and psycho social service programs and the presentation of such programs to physicians, family members, and residents.
