Job summary

Focused Post Acute Care Partners FPACP in Baytown, TX is seeking a Licensed Physical Therapist Assistant to join the Rehab team. Under supervision, you will assist in implementing treatment plans to improve mobility and reduce pain.

Responsibilities include guiding patients through prescribed exercises, documenting progress, and collaborating with the Physical Therapist to adjust plans. The role requires an Associate's Degree in an accredited PTA program and current PTA license.

Qualifications

• Associate's Degree in Physical Therapy Assistant Program ( PTA ).
• Current license/registration as a Physical Therapist Assistant.

Responsibilities

• Assists the Physical Therapist in developing and implementing individual patient care plans.
• Guides patients through prescribed exercise programs to improve strength, mobility and function.
• Records patient response to treatment and overall progress in patient charts.
• Maintains treatment areas and prepares equipment for therapy sessions.
• Collaborates with the PT to adjust treatment plans as needed.
• Participates in staff meetings and follows facility policies.
