Job summary

Focused Post Acute Care Partners FPACP is seeking a Licensed Physical Therapist to join the Therapy department at Linden, TX. The role involves evaluating patients, developing treatment programs with physicians, and delivering physical therapy treatments including modalities and therapeutic exercises.

The PT will supervise any assigned Physical Therapy Assistant, collaborate with other clinicians, and help patients learn activities of daily living while adhering to professional standards.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited college or university.
• Current license/registration by state of hire as a Physical Therapist.

Responsibilities

• Establish, implement, and evaluate treatment plans, goals, and discharge plans for patients.
• Provide direct patient care to ensure the effectiveness of physical therapy programs.
• Apply heat, light, water, and electrical modalities as prescribed.
• Supervise Physical Therapy Assistants when assigned.
• Coordinate care with physicians and other health care providers.
