Job summary

HTS is seeking a Licensed Physical Therapist to join our home health team in Columbus, Indiana, providing therapy services to residents in their homes or at a partner senior living facility. The role offers flexible scheduling and opportunities for growth toward part-time or full-time status.

Applicants must hold an active Indiana Physical Therapist license or be eligible to obtain one; a Doctor of Physical Therapy or equivalent is preferred.

Qualifications

• Active Indiana Physical Therapist licensure or eligibility to obtain Indiana license.
• Advanced degree from an ACPE-accredited Physical Therapy program.
• Eligibility to work in the United States.

Responsibilities

• Assess patients' functional abilities and develop personalized treatment plans.
• Monitor and document patient progress; adjust plans as needed.
• Educate patients and families on exercises and strategies for ongoing health.
• Collaborate with healthcare teams, caregivers, and facility staff.
• Maintain accurate documentation compliant with regulatory standards.
