Focused Post Acute Care Partners is seeking a Licensed Physical Therapist Assistant to support a dynamic rehabilitation team in our Operations department. Under supervision, you will implement treatment plans to help patients regain mobility, increase strength, and reduce pain.

You will guide patients through prescribed exercises, apply modalities as ordered, document progress, and communicate with the supervising Physical Therapist to adjust plans as needed.

Competenze

• Associate's Degree in Physical Therapy Assistant Program required.
• Current license/registration as Physical Therapist Assistant in state of hire.
• Ability to work under supervision of Physical Therapist and follow treatment plans.

Mansioni

• Assist Physical Therapist in developing and implementing patient care plans.
• Provide daily care and supervision of assigned patients.
• Guide patients through prescribed exercise programs to improve strength and mobility.
• Apply modalities and therapeutic interventions as prescribed.
• Document patient progress and communicate with supervising PT.
• Schedule patients and maintain daily census of therapy patients.
• Maintain infection control and equipment cleanliness.
