Prismhr Hire invites a Licensed Physical Therapy Assistant to join our healthcare team across inpatient facilities, outpatient clinics, home health and skilled nursing settings.

The role involves collaborating with licensed therapists to develop and implement effective care plans aimed at improving patient outcomes, educating patients, and supporting discharge planning while ensuring safety and high standards of care.

Qualifications

• Valid license as a Physical Therapy Assistant with current state licensure.
• Experience in outpatient clinics, inpatient settings or home health preferred.
• Skilled nursing facility experience is a plus.
• Familiarity with physiotherapy techniques across orthopedics, geriatrics, pediatrics and acute care.
• Knowledge of medical terminology, physiology, anatomy, and care plan development.
• Ability to work effectively in multidisciplinary teams and communicate clearly with patients of all ages.
• Experience with discharge planning processes is advantageous.

Responsibilities

• Assist licensed physical therapists in implementing individualized treatment plans across inpatient, outpatient, home health and SNF settings.
• Conduct therapeutic exercises and modalities, including trigger point therapy and manual techniques to relieve pain and improve mobility.
• Monitor patient progress and adjust treatment approaches under supervision.
• Educate patients on exercises, injury prevention and recovery strategies.
• Document patient responses and progress in medical records with confidentiality.
• Support discharge planning and prepare patients for ongoing care needs post-treatment.
• Ensure safety by following protocols and maintaining equipment hygiene.
• Collaborate with multidisciplinary teams to optimize patient care and transitions between settings.
