Bebashi - Transition to Hope, a leading nonprofitorganization serving Philadelphia’s diverse and underservedcommunities, is seeking a compassionate, mission-driven LicensedProfessional Counselor (LPC) to join our Wellness Center team. Thisrole will report to Director of Clinical Services.

Founded in 1985 as the first Black-founded and Black-ledorganization in the U.S. to address the HIV/AIDS crisis, Bebashi -Transition to Hope now operates as a comprehensive health and socialservices agency. Our mission is to empower people to enhance thequality of their health and overall well-being by providing accessto high-quality healthcare, education, and social services.
Overview:
Our wellness centers provide low-barrier, culturally competentprimary care and preventive health services for uninsured,underinsured, and underserved individuals—particularly LGBTQ+ andBIPOC residents of Philadelphia.
Major Duties and Responsibilities:
- Provide individual, group, and family counseling for clientsreferred through Bebashi’s Wellness Center and case management programs.
- Conduct behavioral health assessments and screenings (PHQ-9,GAD-7, ACEs) to identify and address patient mental health needs.
- Develop and monitor personalized treatment plans incollaboration with clients, ensuring goals align with overallwellness and primary care outcomes.
- Deliver trauma-informed, culturally competent, andstrengths-based therapy tailored to the needs of BIPOC and LGBTQ+ clients.
- Provide crisis intervention, de-escalation, and safetyplanning for clients in acute distress.
- Collaborate with medical providers, case managers, andprevention specialists to ensure integrated, holistic care coordination.
- Maintain detailed, compliant, and timely documentation in theElectronic Medical Record (EMR) system.
- Facilitate referrals to higher levels of care, includingpsychiatry and substance use treatment providers.
- Participate in case conferences, quality improvement meetings,and multidisciplinary team huddles.
- Provide staff education on behavioral health topics andcontribute to community mental health outreach.
- Hands-On Clinician: Willingness to engage directly in clientcare while supporting larger organizational goals.

Working Location and Conditions:
- Primarily onsite at Bebashi’s Primary Wellness Center (4101Woodland Ave) with some travel to our North Philly Location (1217Spring Garden)
- Monday-Friday, 9:00 AM-5:00 PM; occasional evenings orweekends for outreach or client needs.
- May include limited telehealth services as approved.
- Regular supervision and access to professional developmentresources provided.
Experience and Skills:
Experience and Qualifications
Education:
- Master’s degree in Counseling, Psychology, Clinical MentalHealth Counseling, or related field (required).
Licensure:
- Current, unrestricted Pennsylvania LPC license in goodstanding (required).
Experience:
- Minimum of 3 years of post-master’s counseling experience in aclinical or community health setting.
- Demonstrated experience providing trauma-informed care toBIPOC and LGBTQ+ populations.
- Familiarity with integrated primary and behavioral healthmodels preferred.
- Knowledge of Medicaid, Medicare, and other payer documentationstandards strongly preferred.
- Proficiency with Microsoft Office Suite and EMR systems(eClinicalWorks preferred).
Preferred:
- Bilingual (English/Spanish).
- Training or certification in trauma, substance use counseling,or crisis intervention.
