Job summary

Bebashi - Transition to Hope in Philadelphia, PA is seeking a licensed professional counselor (LPC) to join our Wellness Center team in a full-time, permanent role reporting to the Director of Clinical Services.

The LPC will provide individual, group, and family counseling, perform behavioral health assessments, develop treatment plans, deliver trauma-informed care, and collaborate with medical and case-management staff to coordinate integrated care.

Qualifications

• Master's degree in Counseling or related field required.
• Current PA LPC license in good standing required.
• Minimum 3 years post-master's counseling experience.
• Experience delivering trauma-informed care to BIPOC and LGBTQ+ populations.
• Knowledge of Medicaid/Medicare payer documentation preferred.
• Proficiency with EMR systems (eClinicalWorks preferred) and Microsoft Office.

Responsibilities

• Provide individual, group, and family counseling for clients referred through Bebashi's Wellness Center.
• Conduct behavioral health assessments and screenings to identify and address patient needs.
• Develop and monitor personalized treatment plans in collaboration with clients.
• Deliver trauma-informed, culturally competent therapy tailored to client needs.
• Provide crisis intervention, de-escalation, and safety planning as needed.
• Collaborate with medical providers, case managers, and prevention staff for integrated care.
• Maintain detailed, compliant documentation in the EMR system.
• Facilitate referrals to higher levels of care, including psychiatry and substance use treatment.
• Participate in case conferences and quality improvement meetings.
• Provide staff education on behavioral health topics and contribute to outreach.

Overview

Our wellness centers provide low-barrier, culturally competent primary care and preventive health services for uninsured, underinsured, and underserved individuals—particularly LGBTQ+ and BIPOC residents of Philadelphia.

Major Duties And Responsibilities

• Provide individual, group, and family counseling for clients referred through Bebashi's Wellness Center and case management programs.
• Conduct behavioral health assessments and screenings (PHQ-9, GAD-7, ACEs) to identify and address patient mental health needs.
• Develop and monitor personalized treatment plans in collaboration with clients, ensuring goals align with overall wellness and primary care outcomes.
• Deliver trauma-informed, culturally competent, and strengths‑based therapy tailored to the needs of BIPOC and LGBTQ+ clients.
• Provide crisis intervention, de‑escalation, and safety planning for clients in acute distress.
• Collaborate with medical providers, case managers, and prevention specialists to ensure integrated, holistic care coordination.
• Maintain detailed, compliant, and timely documentation in the Electronic Medical Record (EMR) system.
• Facilitate referrals to higher levels of care, including psychiatry and substance use treatment providers.
• Participate in case conferences, quality improvement meetings, and multidisciplinary team huddles.
• Provide staff education on behavioral health topics and contribute to community mental health outreach.

Working Location and Conditions

• Primarily onsite at Bebashi's Primary Wellness Center (4101 Woodland Ave) with some travel to our North Philly Location (1217 Spring Garden)
• Monday-Friday, 9:00 AM-5:00 PM; occasional evenings or weekends for outreach or client needs.
• May include limited telehealth services as approved.
• Regular supervision and access to professional development resources provided.

Education

• Master's degree in Counseling, Psychology, Clinical Mental Health Counseling, or related field (required).

Licensure

• Current, unrestricted Pennsylvania LPC license in good standing (required).

Experience

• Minimum of 3 years of post-master's counseling experience in a clinical or community health setting.
• Demonstrated experience providing trauma‑informed care to BIPOC and LGBTQ+ populations.
• Familiarity with integrated primary and behavioral health models preferred.
• Knowledge of Medicaid, Medicare, and other payer documentation standards strongly preferred.
• Proficiency with Microsoft Office Suite and EMR systems (eClinicalWorks preferred).

Preferred

• Bilingual (English/Spanish).
• Training or certification in trauma, substance use counseling, or crisis intervention.
