Job summary

Bebashi - Transition to Hope in Philadelphia, PA seeks a Nurse Practitioner to join its Wellness Center team. This licensed clinician will assess patients, develop care plans, order tests, and collaborate with physicians to provide high-quality care to underserved communities.

Required: RN licensure and NP certification, DEA registration if prescribing, and 2–3 years of clinical experience (HIV care preferred). Per diem, exempt role; reports to the Director of Clinical Services.

Qualifications

• Current RN licensure and PA Nurse Practitioner Certification required.
• Master’s or Doctorate in Nursing from an accredited program.
• Current DEA registration (if prescribing) and BLS/ACLS certification.
• Minimum of 2/3 years of clinical experience preferred, HIVHealthcare preferred.

Responsibilities

• Plans patient care based on in-depth knowledge of the patient population and protocol, considering cultural background and support systems.
• Serves as a patient advocate.
• Writes admission, transfers, and discharges orders.
• Orders and interprets laboratory and diagnostic studies.
• Orders of appropriate medication and treatments.
• Refers patients for consultation when indicated (e.g., neurology, ophthalmology, endocrine, surgery, ICU, infectious disease, hematology, psychiatry, social service, dietary).
• Documents cases through progress notes and summaries.
• May perform invasive procedures upon competency.
• Provides contraceptive education, pregnancy planning counseling, and STD testing/treatment.
• Participates in patient care rounds and conferences.
• Communicates patient management strategies to the Clinical Director.
• Collaborates with Bebashi staff to develop management strategies while ensuring HIPAA compliance.
• Recognizes emergencies and initiates life-saving procedures when necessary.
• Collaborates with stakeholders to evaluate and implement new care patterns.
• Participates in outside activities for professional growth.
• Facilitates patient/family conferences to review goals and post-SNF needs.
• Supports decisions re: goals of care, palliative care, and hospice.
• Prescribes medication to patients based on state practice.
• Performs other duties as assigned.

Bebashi - Transition to Hope, a leading nonprofitorganization serving Philadelphia’s diverse and underservedcommunities, is seeking a compassionate, mission-driven LicensedProfessional Counselor (LPC) to join our Wellness Center team. Thisrole will report to Director of Clinical Services.

Position Summary:

Independently assesses acute and non-acute clinical problems.

Performs and documents physical assessments and patienthistories, analyzes trends in patient conditions, and developsdocuments, and implements a patient management plan in response tothe data obtained. This also includes facilitating and collaboratingwith clinical staff for the development of the plan of care as wellas providing appropriate patient/ family/significant othercounseling and education.

Duties/Responsibilities:

Plans patient care based on in-depth knowledge of thespecific patient population and/ or protocol, anticipating and identifying physiological and/ or psychological problems commonlyencountered including the consideration of the patient’s culturalbackground, level of understanding, personality, and support systems.

13. 13. Collaborates with members of the Bebashi staff whenappropriate while ensuring HIPPA compliance to develop patient management strategies that are successful in meeting patient care needs.
14. 14. Recognizes situations that require the immediate attention of a physician, and initiates life-saving procedures when necessary.
15. 15. Uses advanced communication skills to problem solve complexsituations and to improve processes and service to patients.
16. 16. Collaborates with internal and external stakeholders toanalyze and evaluate current systems of health care delivery toidentify and implement new practice patterns as appropriate.
17. 17. Participates in outside activities that enhance personal andprofessional growth and development.
18. 18. Facilitates patient/family conferences to review treatmentgoals, optimize resource utilization, provide family education andidentify post-SNF needs.
19. 19. Enhances a collaborative relationship to maximize thepatient’s/family’s ability to make informed decisions re: goals ofcare, palliative care and hospice.
