Job summary

Premier Home Health Providers Inc. is seeking an experienced Licensed Practical Nurse (LPN/LVN) for per diem home visits in California. You will deliver direct patient care in the home, follow care plans, and support recovery in a comfortable, patient-centered environment.

You will collaborate with the care team, educate patients and families, document care accurately, and maintain safety with aseptic technique. A valid California license, CPR, and reliable transportation are required.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
