Job summary

Madrona Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN) to provide compassionate, in-home hospice care.

You will implement care plans, educate families, document care, and coordinate with RNs and the care team to ensure patient comfort in home settings. Candidates must hold an active CA LPN/LVN license, CPR certification, and a valid driver’s license, with at least 3 years of nursing experience, preferably in home health or med/surg.

Qualifications

• Licensed practical or vocational nursing degree and current California LPN/LVN license.
• Minimum 3+ years of nursing experience, home health or med/surg preferred.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle for home visits.
• Strong communication and collaboration with care team.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team including OT, PT, HHA, and others.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $50 - $60 per visit.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.

• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.
