Job summary

Madrona Hospice Care Inc. is seeking a Licensed Practical Nurse/LPN to provide compassionate, in-home hospice care in California. This per diem role supports patients in their own homes, following individualized plans and collaborating with a multidisciplinary team.

Responsibilities include direct patient care, education for patients and families, and accurate documentation. Travel to patient homes is required; CPR certification and a valid CA license are essential for this important role.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team - including OT, PT, HHA, and other disciplines - and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
