Job summary

Madrona Hospice Care Inc. is seeking a Licensed Practical/Vocational Nurse to provide direct patient care in home hospice settings across California. You will implement care plans, educate families, and document care with a patient-first mindset.

Per diem role offering $50–$60 per visit, with mileage reimbursement, paid sick days, paid bereavement leave, and opportunities for skills training. Travel to patient homes required; Driver’s license and CPR certification mandatory.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
