Job summary

Madrona Hospice Care, Inc. in California is seeking a licensed Practical/Vocational Nurse to provide compassionate home hospice care on a per diem basis. You will help patients remain safe, comfortable, and supported in their own homes, following individualized care plans created by RNs.

The role emphasizes teamwork, empathy, and high-quality service to ease transitions for patients and families. A California LPN/LVN license, CPR certification, and the ability to travel to patient homes are

Qualifications

• Licensed Practical/Vocational Nurse licensed in California.
• 3+ years nursing experience, home health or med/surg preferred.
• Current CPR certification with AHA.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $50 - $60 per visit.

Responsibilities

• Perform other duties as required for patient services.

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg background preferred.
• Current CPR certification with AHA.
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes.
• Strong communication skills and a collaborative, patient-first mindset.
