Job summary

Affinity Hospice Care Inc. is seeking a licensed practical/vocational nurse to join our compassionate home hospice team on a per diem basis in California.

Your role is to provide direct patient care, educate families, and ensure safe, high-quality home-based services. You will work with an interdisciplinary team, follow individualized care plans, document diligently, and travel to patient homes using an insured vehicle.

Qualifications

• Graduate of an accredited practical or vocational nursing program.
• Active LPN or LVN license in California.
• 3+ years of nursing experience; home health or med/surg preferred.
• Current CPR certification with AHA.
• Valid driver’s license and insured vehicle; ability to travel to patient homes.
• Strong communication skills and patient-first mindset.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and follow individualized care plans established by the RN.
• Educate patients and their families on self-care techniques and diagnosis-related information.
• Document care accurately and in a timely manner.
• Collaborate with the care team — including OT, PT, HHA, and other disciplines — and communicate patient needs to the RN or Clinical Supervisor.
• Use supplies and equipment safely, adhering to aseptic technique.
• Perform other duties as required for patient services.
