Mosaic Nursing is hiring a dedicated RN to provide critical care coordination and clinical support in a dynamic care team. The role blends direct patient care with medical oversight, teaching, and care planning to ensure proactive healthcare delivery.

You will work 7p-7a (36 hours/week) with every other weekend, requiring strong critical thinking, communication, and adaptability across diverse populations.

Qualifications

• RN with BSN or ASN from an accredited nursing program.
• Active state nursing license in good standing; valid driver's license required.
• 1-2 years of general nursing experience; experience with IDD valued.
• Current CPR and First Aid (FA) certifications.
• Proficiency with computers/technology for daily documentation and task management.
• Physical ability to push, pull, and lift at least 50 lbs.

Responsibilities

• Medical Monitoring: review daily nursing documentation, identify health trends, support quality audits.
• Care Coordination: coordinate with pharmacies/doctors, complete medical appointments, advocate at care conferences.
• Clinical Support: provide hands-on care including wound/ostomy/catheter management.
• Education & Instruction: teach medication administration, lead clinicals for CMAs/Medication Managers, staff instruction on care plans.
• Administration: update care plans and protocols; use Therap for documentation and compliance.

You will act as a medical guide during crises and a consistent advocate for individuals during critical care appointments.

In this role, you aren't just a nurse; you are a teacher and a coordinator who builds lasting relationships with medical providers and families to ensure well-managed, proactive healthcare is received.

What YOU'LL Do:

• Medical Monitoring: Monitor daily documentation for medical needs, identify health trends promptly, and perform health monitoring of documentation to ensure high scores in quality audits
• Care Coordination: Coordinate with pharmacies and doctors, complete medical appointments, and provide advocacy at critical care consultations
• Clinical Support: Provide hands-on care when needed, including wound care, ostomy care, catheter management, and monitoring critical care medical needs.
• Education & Instruction: Teach medication administration refresher courses, lead clinicals for CMAs and Medication Managers, and provide on-site instruction for staff regarding care plans and medical protocols
• Administration: Update and maintain care plans and medical protocols while utilizing technology (Therap) for accurate documentation and regulatory compliance.

SCHEDULE: 7p-7a (36hrs/week), every other weekend

• Physical Requirements: Ability to push, pull, and lift at least 50 lbs.
• Soft Skills: Strong critical thinking, time management, and the ability to teach individuals from diverse cultures and learning styles.
