Job summary

Remote Care Partners, Inc. is seeking a Virtual Nurse Coach (LPN) for a remote, full-time role. You will educate patients on remote monitoring equipment, review vital signs, and collaborate with providers to develop patient-centered care plans.

The role emphasizes proactive care, patient coaching, and preventing urgent care visits. The position requires at least two years of nursing experience, active state licensure, compact licensure, EMR proficiency, and strong computer skills.

Qualifications

• Two years of nursing experience; case management or home health preferred.
• Active state licensure and Compact Licensure.
• EMR experience and computer competency.
• Experience caring for diabetes, COPD, asthma, CHF or A Fib.

Responsibilities

• Welcoming patients to the benefits of remote monitoring.
• Educating patients on equipment they will use.
• Reviewing patient vital signs in near real time.
• Obtaining actionable information from patients with abnormal readings.
• Escalating patient situations to the provider via the EMR.
• Assisting the provider in creating patient-centered care plans.
• Monitoring notifications and observing trends.
• Creating and reviewing monthly care plans with patients.
• Monitoring patient adherence to plans and devices.
• Coaching patients on healthier behaviors.
• Motivating factors to improve health outcomes and proactively managing care to prevent urgent visits.

Virtual Nurse Coach - LPN - Remote location!

The Role: Remote Virtual Nurse Health Coach

Key Job Functions:

• Welcoming patients to benefits of remote monitoring
• Educating patients on equipment they will use
• Near real time review of patient vital signs
• Obtaining actionable information from patients with abnormal readings
• Using practice EMR to escal patient situations to the provider for medical intervention
• Reviewing & assisting the provider in the creation of a patient centered care plan
• Monitoring notifications and observing trends
• Creating patient centered care plans as well as reviewing monthly plans with patients
• Monitoring patient compliance with the plan of care & devices
• Coaching patients on healthier behaviors
• Act as first contact for patient motivationg factors and leveraging them for best possible health outcomes
• Proactively managing patients to prevent urgent care or emergency room visits

Required Experience:

• 2 years nursing experience; case management or home health preferred
• Must have active state licensure
• Must have active Compact Licensure
• EMR Experience
• Computer competency
• Experience caring for some or all of following conditions:
• Diabetes
• COPD
• Asthma
• CHF
• A Fib
