Job summary

Remote Care Partners, Inc. is seeking a Virtual Nurse Coach (LPN) to provide essential remote patient support. You will educate patients on equipment, review vitals in near real time, and escalate situations via the EMR to the provider for intervention.

You will help create and review patient-centered care plans, monitor trends and adherence, coach healthier behaviors, and work from home in a full-time, salary role with hardware/software provided.

Qualifications

• 2 years nursing experience; case management or home health preferred.
• Must have active state licensure and Compact Licensure.
• EMR experience and computer competency.
• Experience caring for Diabetes, COPD, Asthma, CHF, or A Fib.

Responsibilities

• Welcoming patients to benefits of remote monitoring.
• Educating patients on equipment they will use.
• Near real time review of patient vital signs.
• Obtaining actionable information from patients with abnormal readings.
• Using EMR to escalate patient situations to the provider for medical intervention.
• Reviewing & assisting the provider in the creation of a patient-centered care plan.
• Monitoring notifications and observing trends.
• Creating patient-centered care plans and reviewing monthly plans with patients.
• Monitoring patient compliance with the plan of care & devices.

• Act as first contact for patient motivating factors and leveraging them for best possible health outcomes.
• Proactively managing patients to prevent urgent care or emergency room visits.

Key Job Functions:

Required Experience:
