Job summary

Remote Care Partners, Inc. seeks a Virtual Nurse Coach (LPN) for a remote, full-time role. You will welcome patients to remote monitoring, educate on devices, review vitals in real time, and escalate when needed.

You’ll work with providers to create patient-centered care plans and monitor adherence to the care plan and devices. The ideal candidate has 2 years of nursing experience with case management or home health, active LPN licensure (compact where applicable), EMR proficiency, and strong

Qualifications

• 2 years nursing experience; case management or home health preferred
• Active state licensure required
• Active Compact Licensure required
• EMR experience
• Computer competency
• Care for diabetes, COPD, asthma, CHF, or A Fib conditions as applicable

Responsibilities

• Welcoming patients to the benefits of remote monitoring
• Educating patients on equipment they will use
• Reviewing patient vital signs in near real time
• Escalating patient situations to the provider when needed
• Assisting in creating patient-centered care plans
• Monitoring notifications and observing trends
• Developing and reviewing monthly care plans with patients
• Ensuring patient adherence to the plan of care and devices
• Coaching patients on healthier behaviors
• Motivating patients to achieve health outcomes
• Proactively managing patients to prevent urgent care or ER visits

Virtual Nurse Coach - LPN - Remote location!

The Role: Remote Virtual Nurse Health Coach

Key Job Functions:

• Welcoming patients to benefits of remote monitoring
• Educating patients on equipment they will use
• Near real time review of patient vital signs
• Obtaining actionable information from patients with abnormal readings
• Using practice EMR to escal patient situations to the provider for medical intervention
• Reviewing & assisting the provider in the creation of a patient centered care plan
• Monitoring notifications and observing trends
• Creating patient centered care plans as well as reviewing monthly plans with patients
• Monitoring patient compliance with the plan of care & devices
• Coaching patients on healthier behaviors
• Act as first contact for patient motivationg factors and leveraging them for best possible health outcomes
• Proactively managing patients to prevent urgent care or emergency room visits

Required Experience:

• 2 years nursing experience; case management or home health preferred
• Must have active state licensure
• Must have active Compact Licensure
• EMR Experience
• Computer competency
• Experience caring for some or all of following conditions:
• Diabetes
• COPD
• Asthma
• CHF
• A Fib

Location: Remote, working from home
