Job summary

Remote Nurse Health Coach (LPN) - Virtual, fully remote. Welcome patients to remote monitoring and educate them on the devices used. This is a full-time salary role with software and hardware provided.

Requirements: 2 years nursing experience, active state licensure, Compact licensure, EMR experience, and solid computer skills. Diabetes/COPD/Asthma/CHF/A Fib experience is a plus; you will help create patient-centered care plans and monitor progress.

Qualifications

• 2 years nursing experience; case management or home health preferred.
• Must have active state licensure.
• Must have active Compact Licensure.
• EMR experience.
• Computer competency.
• Experience caring for: Diabetes, COPD, Asthma, CHF, A Fib.

Responsibilities

• Welcoming patients to benefits of remote monitoring.
• Educating patients on equipment they will use.
• Near real time review of patient vital signs.
• Obtaining actionable information from patients with abnormal readings.
• Using practice EMR to escalate patient situations to the provider for medical intervention.
• Reviewing & assisting the provider in the creation of a patient centered care plan.
• Monitoring notifications and observing trends.
• Creating patient centered care plans and monthly reviews with patients.
• Monitoring patient compliance with the plan of care & devices.
• Coaching patients on healthier behaviors.
• Act as first contact for patient motivation factors and leveraging them for best possible health outcomes
• Proactively managing patients to prevent urgent care or emergency visits.
