Seven Hills Foundation in Fitchburg, MA is seeking an LPN Supervisor to lead the nursing team, provide direct care, and ensure compliance with DDS standards across designated programs.

You will mentor staff, oversee onboarding and training, and coordinate nursing coverage while ensuring accurate documentation in the EHR and timely communication with RNs and leadership.

Qualifications

• LPN license is required and active in Massachusetts.
• Experience supervising nursing staff is preferred.
• Proficiency in administering medications per physician orders and protocols.
• Familiarity with DDS standards and agency policies.

Responsibilities

• Provide high-quality nursing care and medication administration to individuals per orders and protocols.
• Serve as a clinical resource and mentor to LPNs and direct care staff.
• Assist in onboarding, orientation, and training of nursing staff and oversee clinical competencies.
• Monitor health care plans and treatment protocols for compliance.
• Coordinate nursing coverage across programs to ensure adequate staffing.
• Communicate health issues promptly to RNs and program leadership.
• Document health assessments and care in Electronic Health Records (EHR).
• Accompany individuals to medical appointments and ensure follow-up care is completed.

Job description

Pay: $42.00-$44.00/hour

Schedule: Wednesday 9am-5pm, Thursday 7am-11pm, Friday 9am-5pm; Mandatory staff meeting every 3rd Wednesday of the month 1pm-3pm.

Licensed Practical Nurse (LPN) Supervisor

The Licensed Practical Nurse (LPN) Supervisor plays a critical leadership role within the nursing team, providing direct care to individuals while supporting the oversight and coordination of healthcare services across designated programs. The LPN Supervisor ensures high-quality medical support in accordance with healthcare regulations, organizational standards, and the Nurse Practice Act. This position also supports supervision and training of other nursing staff, contributes to clinical decision-making, and ensures compliance with all medication and treatment protocols.

What You'll Do

• Provide high-quality nursing care and medication administration to individuals in accordance with physicians' orders and agency protocols
• Serve as a clinical resource and mentor to other LPNs and direct care staff within assigned programs
• Support the onboarding, orientation, and training of nursing staff, including oversight of clinical competencies
• Monitor the implementation of health care plans and treatment protocols
• Assist in the scheduling and coordination of nursing coverage across programs, ensuring consistent and adequate staffing
• Communicate medical issues promptly and effectively to RNs, Health Care Directors, physicians, and program leadership
• Conduct or assist in health assessments and ensure timely documentation in Electronic Health Records (EHR)
• Accompany individuals to medical appointments when necessary, and ensure follow-up care is completed and documented
• Monitor and maintain compliance with Medication Administration Program (MAP) standards and provide MAP training as required
• Participate in incident reporting, follow-up, and resolution in collaboration with clinical and administrative leadership
• Ensure health records and documentation are complete, accurate, and compliant with DDS and agency standards
• Promote a culture of respect, safety, and dignity in all interactions with individuals and staff
• Perform other nursing or administrative duties as assigned by the supervising RN or Health Care Director
