Job summary

Harmony community in Murfreesboro, TN is seeking an LPN to manage medication plans, assess resident needs, and coordinate with physicians and nursing staff. You will support wellness, safety, and nutrition while documenting care and outcomes.

Ideal candidates hold an active LPN license in TN, have senior-care experience, and demonstrate teamwork, communication, and time-management skills. Flexible 8–16 hour shifts including weekends/holidays are required.

Qualifications

• Current LPN license to practice nursing in the state where the community is located.
• Experience working with senior population, preferably in assisted living or LTC.
• Must be a team player who works well with others and supervises staff.
• Strong communication skills, both verbal and written.
• Good time management and organizational abilities.
• Ability to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
• Willingness to work a flexible 8- to 16-hour schedule including weekends/holidays.
• Must complete required trainings and certifications as required by the state.

Responsibilities

• Execute the medication management plan with periodic reviews and physician communication when changes are needed.
• Continuously assess resident needs and update care plans and documentation.
• Implement measures to ensure resident wellness and safety with proper documentation.
• Work collaboratively with the team to meet resident needs.
• Respond to emergencies promptly and calmly.
• Report incidents and changes in resident condition with documentation.
• Participate in activities and community events.
• Assist with meal delivery and monitor nutrition and diet adherence.
• Maintain high standards for record-keeping and documentation.
• Establish positive relationships with all departments.
• Identify risk factors and initiate appropriate interventions and documentation.
• Provide status reports to the nursing team during shift changes and in the Daily Log.
• Provide motivation and direction to CNAs/RMAs.
• Be certified or willing to obtain required state certification.
• Attend trainings, CEUs, and staff meetings.
• Adhere to policies and procedures and proper body mechanics.
• Maintain attendance and arrive ready to work.
• Complete other duties as assigned.
