Job summary

Interim HealthCare is seeking a Private Duty LPN or RN for a full-time role in Knoxville, TN. The position runs Monday–Friday, 7:30am–3:30pm, with one-on-one patient care in the home and off every weekend.

Qualifications

• Must have completed an accredited practical nursing program and hold TN RN or LPN license.
• CPR certification required, with knowledge of home health regulations.
• Strong communication and compassionate patient care are essential.

Responsibilities

• Provide home-based care to patients with illnesses or chronic diseases, often after hospital discharge, under RN supervision.
• Collaborate with physicians, RNs, CNAs, and aides to deliver quality care.
• Take vitals, perform assessments, document progress, and report changes to RN supervisor.
• Assist with patient care plan execution and teach self-care techniques to patients.
• Educate families on care plans, medications, and home safety.
• Coordinate with interdisciplinary team for discharge planning.

As a Home Health Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
