Job summary

Lake Park of Madison Nursing and Rehabilitation Center is seeking a self-directed professional to oversee all aspects of resident care and ensure continuous delivery of high-quality services.

In this role you will work with families and multiple departments to provide person-centered care across Sub-Acute Rehab and Long Term Care settings. The position features a 7am–7pm rotation with additional PRN shifts.

A Florida nursing license is required, and flexibility is essential to meet patient needs.

Qualifications

• Current Florida nursing license.

• Experience in dementia care, respiratory care, post-operative care, dialysis care, wound care and end-of-life care recommended.

Responsibilities

• Oversee all aspects of resident care and service delivery.
• Collaborate with Therapy, Dietary, and Social Work teams to ensure comprehensive person-centered care.
• Manage and supervise the care team with critical thinking.
• Cover 7am–7pm rotations; PRN shifts available.
• Coordinate Sub-Acute Rehab and Long Term Care services.

This position requires a self-directed, responsible individual who will oversee all aspects of resident care and assure continued delivery of service.

Responsibilities and Duties

• As a member of the team, this position requires critical thinking skills and the ability to demonstrate a level of proficiency in the management/oversight of their team.

• Interaction with families, departments such as Therapy, Dietary and Social Work to ensure comprehensive person centered care is delivered is an essential part of the position

• Sub-Acute Rehab as well as Long Term Care

• OPEN POSITION- 7am - 7 PM scheduled rotation

• All shifts available PRN

Qualifications and Skills

• Current license in Florida

• Qualified candidates would be required to be flexible in their schedule
