Senior Lifestyle Corporation is seeking a Licensed Practical Nurse (LPN) for the Evergreen Retirement Community in Cincinnati, OH. This full-time, on-site role supports residents in an Assisted Living setting, delivering licensed nursing care and medication administration under established guidelines.

Responsibilities include admissions assessments, care plan development, clinical documentation, emergencies response, and interdepartmental communication.

Qualifications

• Active LPN license in the practice state.
• Minimum 1+ year experience in senior care or related field.
• CPR certification is required.
• Proficient in English for duties and emergencies.
• Basic computer skills for charting and documentation.

Responsibilities

• Administer medications in accordance with Federal & State regulations and company policies.
• Participate in admissions assessments and documentation for new residents.
• Contribute to development and maintenance of acute and long-term care plans.
• Track clinical information and documentation as directed (wounds, infection control).
• Provide health-related consultations within the community and notify physicians/families of changes.

Do you have HEART? This position is responsible for providing licensed nursing services to residents, following the guidelines provided by Senior Lifestyle Corporation.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Years of Experience: 1+ years
Type of Experience: Senior care or related field
Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
