Senior Lifestyle Corporation seeks a licensed Practical Nurse (LPN) to provide nursing services in our assisted living community in Albuquerque. The role emphasizes compassionate care, medication administration, admissions assessments, and developing care plans for residents.

Requires an active LPN license in the state, CPR certification, and at least 1 year of senior care experience. The position is full-time, with team-oriented responsibilities and opportunities to educate residents and staff

Qualifications

• Current Licensed Practical Nurse license in the state where practicing.

• 1+ year experience in senior care or related field.

Responsibilities

• Administer medications in compliance with regulations and guidelines.
• Participate in admissions assessments and documentation for new residents.
• Develop and maintain acute and long-term care/service plans.
• Coordinate with clinical staff on wound/infection tracking and other conditions.
• Provide health-related guidance and education to residents and staff.
• Notify physicians or family of health changes and document accordingly.
• Respond promptly to emergencies and maintain clear department communication.
• Discharge planning, assessments, and clinical documentation as needed.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.


• May be responsible for the admissions of new residents to include all admission assessments and documentation.


• Responsible for completing acute and long-term care plans or service plans.


• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.


• Provide general consultation regarding health-related concerns within the community.


• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.


• Respond quickly to all emergencies.


• Maintain close communication with all departments and staff members.


• Provide educational meetings for residents and staff.


• Maintain confidentiality of residents’ and prospective residents’ information.


• May be responsible for discharges, assessments, and clinical documentation as needed.

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified
Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
