Job summary

Senior Lifestyle is seeking a Licensed Practical Nurse to provide licensed nursing services to residents in our West Chester community. You will administer medications, perform admissions assessments, and contribute to care plans under guidance from the director.

The role requires 1+ year in senior care, an active LPN license in the practicing state, CPR certification, and proficient English for communication with staff, residents, and families.

Qualifications

• Must have Licensed Practical Nurse license current in the state of practice.
• CPR certification required.
• English proficiency to perform duties and respond in emergencies.

Responsibilities

• Administer medications in accordance with federal/state regulations and company guidelines.
• Assist with admissions assessments and documentation for new residents.
• Develop and update acute and long-term care plans or service plans.
• Monitor and communicate changes in residents' health to physicians and families.
• Respond promptly to emergencies and coordinate with the team.
• Maintain confidentiality of residents' information and ensure proper documentation.
• Provide education to residents and staff as needed.
• Coordinate with other departments to ensure resident well-being.

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.
