Fairview Hospital / Cleveland Clinic is seeking a Licensed Practical Nurse (LPN) to provide 1-on-1 skilled nursing care in patients' homes for pediatric and adult populations. You will use the nursing process to assess, plan, implement, and evaluate care, administer services per orders, and monitor changes in patients' physiologic and psychosocial status.

Qualifications

• Current LPN/LVN license in the practicing state.
• Current TB or Chest X-Ray.
• Current BLS certification.
• Preferably, one year of professional nursing experience.
