Job description

This Licensed Practical Nurse (LPN) position is located at the Omaha VA Medical Center in Omaha, Nebraska. The LPN works under the supervision of an RN as part of a Primary Care team, providing basic and specialized nursing care for adult and geriatric patients with various physical, mental, behavioral, or psychological needs.

Responsibilities

1. Serve as an LPN on a Patient Aligned Care Team (PACT) in Primary Care.
2. Assist with patient intake and maintain clinic flow.
3. Complete clinical reminders and preventative health screenings.
4. Document all data and nursing care in the electronic medical record.
5. Report relevant patient information to the RN or provider.
6. Prioritize tasks effectively and manage time efficiently.
7. Work collaboratively with team members and communicate tactfully and courteously.
8. Assist providers with exams and procedures.
9. Perform treatments such as electrocardiograms, vital signs, weights, blood glucose readings, basic foot care, bladder scans, ear flushes, etc.
10. Administer immunizations and medications, monitoring for side effects.
11. Participate in patient care management and follow-up.
12. Prepare and adapt procedures to meet patient needs, observing for complications.
13. Recognize deviations in patient conditions and initiate emergency measures, including CPR.
14. Perform venipuncture and specimen collection, and prepare specimens for storage or transport.
15. Use VA computer systems efficiently and safely, following policies.
16. Participate in team huddles and meetings.
17. Maintain positive professional relationships with patients, families, and staff.
18. Perform additional duties as assigned.

Work Schedule and Benefits

Monday - Friday, 8:00 am - 4:30 pm. Competitive salary with regular increases.
