Summary
This Licensed Practical Nurse (LPN) position is located at the Grand Island VA Medical Center in Grand Island, Nebraska. The LPN works under the supervision of an RN as part of a Primary Care team, providing basic and specialized nursing care for adult and geriatric patients with various physical, mental, behavioral, or psychological needs.
Duties

- The selected candidate will work directly with veterans in face-to-face, video, or telephone settings.
- Serve as an LPN on a Patient Aligned Care Team (PACT) in Primary Care.
- Assist with patient intake and maintain clinic flow.
- Complete clinical reminders and preventative health screenings.
- Document all patient data and nursing care in the electronic medical record.
- Report relevant patient information to the RN or provider.
- Prioritize tasks and manage time effectively in the clinical setting.
- Communicate tactfully and courteously with colleagues and patients.
- Assist providers with examinations and procedures as needed.
- Perform treatments such as electrocardiograms, vital signs, weights, blood glucose readings, basic foot care, bladder scans, ear flushes, etc.
- Administer immunizations and monitor for side effects.
- Administer medications as ordered, following policies, and observe for adverse reactions.
- Participate in managing and following up on patient care needs.
- Prepare for procedures, adapting to patient needs, and manage complications.
- Recognize and respond to changes in patient conditions and initiate emergency measures when necessary, including CPR.
- Perform venipuncture and specimen collection, and prepare specimens for transport.
- Use VA computer systems efficiently and safely, adhering to policies.
- Participate in team huddles and meetings.
- Maintain positive professional relationships with patients, families, and staff.
- Perform additional duties as assigned.
